is intended to assist you, the Plan Administrator, in complying with the rules governing plan administration. It is
only a guide. It is intended to help you maintain proper records and
perform other duties that will enable us to complete and file the various
forms and tests referred to in this guide. It will also enable us to
assist you effectively in the event of an IRS or DOL audit of the Plan.

former employee that is requesting a distribution, this Guide should help
you determine what steps you should take. Please take a few minutes to
